5/27/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Jahsim made noises, blurted out, and talking out excessively for the purpose of seeking attention from adult figures/peers. Jahsim began to display playful, silly behaviors as evidenced by making inappropriate noises and looking for attention. 2. Behavior: Jahsim presented symptoms of withdrawn at times within the session as he was playing with his phone and touching other items within the home as he needed multiple redirection from QMHP-C as well as his mother. 1.Intervention:. QMHP-C confronted negative behaviors and explored the purpose of Jahsimâs behaviors. QMHP-C provided Jahsim more positive alternatives to behavior. QMHP-C offered positive feedback for more appropriate behaviors exhibited. QMHP-C reiterated the importance of attempting to work more independently and only seeking assistance when necessary. QMHP-C explained how behaviors are counter-productive to achieving personal and treatment goals, and offered Jahsim more positive productive options to behaviors. 2. Intervention: QMHP-C build therapeutic trust and rapport with consistent eye contact, active listening, unconditional positive regard, and warm acceptance.QMHP-C utilized reward system/verbal praise to reinforce Jahsimâs initiation of verbalization. 1.Response: Jahsim seemed to be nonchalant towards intervention by walking away on several occasions. Jahsim was not focused by looking around and focusing on his surroundings. 2. Response: Jahsim was receptive after several occasions but struggled with interacting and understand himself and his actions developed. Jashim reports to QMHP-C on how he continues to do what he is supposed to be doing in the home but it is others that make him mad so when he is mad he is not going to do anything anyone tells him to do.
